Draping will be used during the session – only the areas being worked on will be uncovered. Clients under 18 must be accompanied by a parent or legal guardian 

during the entire session. Informed written consent must be provided by parent or legal guardian for any client under the age of 18.

Consent for Treatment: If you experience any pain or discomfort during this session, you will immediately inform the practitioner so that the pressure and/or strokes may be 

adjusted to my level of comfort. You further understand that massage/bodywork should not be construed as a substitute for medical examination, diagnosis, or 

treatment and that you should see a physician, chiropractor, or other qualified medical specialist for any mental or physical ailment of which you are aware. You understand 

that massage/bodywork practitioners are not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness, and that 

nothing said during the session given should be construed as such. Because massage/bodywork should not be performed under certain medical conditions, you will have stated all your known medical conditions and answered all questions honestly. Agree to keep the practitioner updated as to any changes in your medical 

profile and understand that there shall be no liability on the practitioner’s part should you fail to do so. You also understand that any illicit or sexually suggestive remarks or 

advances made by you will result in immediate termination of the session, and you will be liable for payment of the scheduled appointment. Understanding all of this, You

give your consent to receive care.

You understand, have read and completed this questionnaire truthfully. You agree that this constitutes full disclosure, and that it supersedes any previous verbal or written 

disclosures. You understand that withholding information or providing misinformation may result in contraindications and/or irritation to the skin from treatments 

received. The treatments you receive here are voluntary and you release this institution and/or skin care professional from liability and assume full responsibility thereof.

Due to our 24hr cancelation policy we need to have you card info on file at the moment you make your appointment. Please fill the card info request thru our secured server. You card will NOT be charged unless you fail to show to you session, reschedule or cancel your appointment 24hr before. The charge in this case will be 50% of the original price of the service scheduled. 

Please have in mind that if you don’t do this in the next 24hrs  appointment it will be cancel.

We will try to send you a reminder the day before your session but it is your responsibility to remember and fail to receive this reminder will not excuse you from missing your appointment.
